Band 6 Community Psychiatric Nurse (CPN) / Care Coordinator
Location: Southend-on-Sea, Essex
Start Date: ASAP
Duration: Initially 4 weeks, with possible extension
Hours: Monday to Friday, 9:00am–5:00pm
Rate: £27.00 per hour gross
Athona is currently recruiting for two experienced Band 6 Community Psychiatric Nurses (CPNs) / Care Coordinators to join the Recovery & Wellbeing Team Hub at Southend.
The successful candidates will be responsible for managing a designated caseload of complex and highly complex patients with severe and enduring mental health needs. You will work collaboratively with the multidisciplinary team to assess, plan, coordinate and deliver person-centred care.
Key Responsibilities:
Manage a caseload of complex and highly complex mental health patients.
Act as Care Coordinator, planning and coordinating care alongside the MDT.
Complete comprehensive assessments using clinical reasoning and evidence-based practice.
Develop and implement individualised care plans and interventions.
Identify and manage clinical risk, working collaboratively with patients, families, carers and partner agencies.
Establish therapeutic relationships and support patients to achieve their health and wellbeing goals.
Monitor and evaluate interventions and treatment outcomes.
Participate in MDT meetings, reviews and care planning discussions.
Maintain accurate clinical documentation and activity records.
Provide clinical leadership and professional expertise within the team.
Support the supervision and development of junior staff and students where required.
Requirements:
Registered Mental Health Nurse (RMN).
Current Band 6 experience.
Strong experience in community mental health and care coordination.
Experience managing complex and highly complex caseloads.
Good knowledge of clinical risk assessment and management.
Excellent communication, organisation and MDT working skills.
