Job Title: Band 6 Case Coordinator

Description
We are currently seeking a Band 6 Case Coordinator to work with our NHS client within the Restormel team in Cornwall. This role involves managing a caseload of patients in the community and requires a full driving licence, as travel between patients may be necessary.
The role is Monday to Friday, 09:00–17:00. Mileage can be claimed for work-related travel. Please note that the Trust is unable to provide accommodation for this booking. This Role also falls inside IR35.
Job Role
Coordinate and manage a caseload of patients within the community.
Support patients in line with their individual care needs.
Work collaboratively with relevant healthcare professionals and community services.
Maintain appropriate patient records and documentation.
Travel within the local community as required to support patient care.
Contribute to effective communication and continuity of care across the team.
Essential for the role
Relevant experience in case coordination or community healthcare.
Full, valid driving licence.
Ability to manage a community caseload effectively.
Strong communication and organisational skills.
Ability to work independently and as part of a multidisciplinary team.
Availability to work Monday to Friday, 09:00–17:00.
