Contract Dates: 5th October 2026 – 31st December 2026 with possible of extension.
Working Hours: Monday to Friday, 09:00–17:00 (some flexibility required)
Job Description
Athona Nursing is currently recruiting experienced Band 6 Continuing Healthcare (CHC) Nurse Assessors for an exciting opportunity across Kent and Medway.
This role involves assessing, planning and coordinating personalised care packages for individuals with complex or long-term health conditions in their own homes and care homes.
You will work collaboratively with multidisciplinary teams, Local Authorities, Primary Care Networks and other healthcare professionals to ensure patients receive appropriate, safe and person-centred care in line with the NHS National Framework for Continuing Healthcare (CHC) and Funded Nursing Care (FNC).
Key Responsibilities
Conduct CHC Checklists, Fast Track Assessments and Decision Support Tool assessments.
Complete Mental Capacity Assessments and contribute to Best Interest Decisions.
Assess patients’ complex care needs and develop personalised care plans.
Liaise with Local Authorities and multidisciplinary teams to support eligibility decisions.
Support the development of Personal Health Budgets and individualised care packages.
Ensure patient safety, safeguarding and accurate clinical documentation.
Communicate effectively with patients, families, carers and healthcare professionals.
Manage complex caseloads independently while working collaboratively within a wider team.
Essential Requirements
Registered Nurse with active NMC registration.
Level 6 degree or equivalent qualification, or equivalent knowledge gained through experience, alongside a nursing diploma.
Experience managing complex caseloads and supporting or mentoring colleagues.
Strong communication, organisational and decision-making skills.
Ability to work independently, manage competing priorities and escalate concerns appropriately.
Knowledge of CHC frameworks, clinical governance and safeguarding principles.
Good IT skills and the ability to maintain accurate clinical records.
