Job Description
Currently, we have an excellent opportunity for an experienced Dementia Care Manager to join a high-quality residential care service supporting older people living with dementia.
This is an exciting opportunity for an experienced care professional to take a lead role in delivering person-centred, relationship-focused dementia care, while supporting and developing a dedicated care team.
Salary: £45,000 per annum
Position: Dementia Care Manager
Hours: 40 hours per week
Setting: Residential Care Home
Location: Chelmsford, Essex
Your key responsibilities will be;
- Lead and oversee high-quality, person-centred dementia care within the service.
- Develop and monitor individual care plans, risk assessments and advanced care plans.
- Champion evidence-based dementia practice and meaningful activities across the dementia community.
- Lead, coach and support care teams, ensuring appropriate staffing, skills and training.
- Build effective relationships with residents, families, GPs and multidisciplinary professionals.
- Monitor quality, safeguarding, health and safety, and regulatory compliance within the service.
- Support audits, inspections, improvement plans and effective management of departmental resources.
For this role, we’re looking for someone who has;
- At least four years’ experience supporting older people, including people living with dementia.
- Supervisory or management experience within a health or social care setting.
- Strong knowledge of dementia care, safeguarding, care planning and end-of-life care.
- Knowledge of CQC requirements, care standards, health and safety, and quality frameworks.
- NVQ/QCF Level 5 qualification, or willingness to work towards the qualification.
