Athona have an exciting opportunity for a band 5/7 Speech & Language Therapist to work in an ongoing locum role based in Cambridgeshire & Peterborough.

  • To provide ESD in-reach to the community beds for patients under the ESD service and work with patients in their own homes.
    * To provide a high standard of Speech and Language Therapy.
    * To perform Speech and Language Therapy assessment of patients and to deliver an individual treatment programme.
    * To hold responsibility for own caseload and be responsible for a defined area of the service working unsupervised.
    * To work as an autonomous practitioner within the multi-professional Early Supported Discharge service for Stroke (ESD) as part of the Countywide Neuro Rehab Service.
    * Senior staff will provide support, regular formal training, clinical reasoning sessions and peer review.
    * To be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients under the care of the Stroke ESD within professional guidelines, with the support of Specialist and Advanced Practitioners within the integrated ESD and Community Neuro Rehab Team.
    * To identify patient needs, agree goals and provide appropriate therapy interventions relating to their communication or swallowing needs. Provide appropriate rehabilitation programmes and make referrals on to other professionals and agencies as appropriate in order to meet the identified needs.
    * To be involved in coordinating and providing 6-month reviews for stroke survivors.
    * To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
    * To allocate work to non-registered staff.
    * To provide neuro advice to professionals from other disciplines.
    * Provide support and education to peers, new staff, non-registered staff and students.
    * To supervise non-registered staff, overseeing patient intervention and ensure that they deliver high quality patient care.
    * To be accountable for the delegated caseload held by non-registered staff in the team.
    * To be actively involved in training, teaching and supporting non-registered staff to achieve competencies.
    * To build up and maintain key relationships with referring services and CNRS to facilitate appropriate referrals and an understanding of the roles within the ESD team.
    * To take an active role in the stroke rehabilitation duty process to prioritise and ensure timely response for assessments.
    * To work in various community settings including the community beds, this involves lone working, with access to Specialist and Advanced Practitioners when required.

Requirements include: HCPC registration, recent experience within the NHS, enhanced DBS, two current references and the right to work in the UK.
