Job Description

Are you an experienced Occupational Therapist looking for an exciting new locum position? We’re working with an outstanding NHS Trust in Nottinghamshire who is looking for an individual with with HCPC registration, recent NHS experience, enhanced DBS and right to work in the UK.

Band 7 Vocational Occupational Therapist (for work In new post long covid team)

You will be an integral member of the Integrated Care Team which delivers therapy across intermediate care, falls and long-term condition management. To provide expert clinical case management for patients living in the community with deteriorating health that may result in declining quality of life or potential hospital admission.

Support patients/clients who want to remain living independently in their own community. To improve or maintain their level of function and quality of life.

The post holder will be able to provide specialist assessment in order to contribute/lead on planned co-ordination of patient care.

To assist in the development and implementation of appropriate pathways of care for people with long term conditions.
Start Date: ASAP
Duration: 31/03/2023
Travel: Yes, the candidate must have their own transport for this role.
